Behavioral Health Specialist

POSITION SUMMARY

This position, in the Level 1 Psychiatric Acute Care unit, assists the primary care provider and LMSW in recognizing, treating, and managing mental health and psychosocial issues and acts as a contributing member of the interdisciplinary team. Provide targeted intervention for social, emotional and environmental issues that impact consumer/family ability to optimally benefit from care.

POSITION DUTIES

  • Completes Intakes for PAC consumers.
  • Completes SMI determination for qualifying PAC consumers.
  • Uses recovery-oriented, trauma-informed and person-first language in all interactions, with a strong understanding and ability to demonstrate excellence in customer service.
  • Actively collaborates with team to meet consumer care, outcome management and system improvement goals.
  • Facilitate family meetings to help ensure safe discharge/coordination of care.
  • Complete and update treatment plans as needed for consumers.
  • Complete ASAMs and referrals to Substance Abuse Inpatient and Outpatient Programs as needed.
  • Participate in daily Rounds with PACU Treatment Team.
  • Participate in A.R.S. Title 36 evaluation process and testify at court proceedings as needed.
  • Coordinate services for consumers with appropriate community agencies as needed.
  • Facilitate discharge planning with consumers, their families, and treatment providers as needed.
  • Provide educational groups for consumers on the unit.
  • Conducts telephone consultation either planned, scheduled intervention contacts or follow-ups with consumer.
  • Work with internal and external customers working towards the continuum of care for consumer.
  • Completes Crisis Prevention Plans with consumers in PAC.
  • Use of recovery-oriented, person-first language in all interactions, with a strong understanding and ability to demonstrate excellence in customer service.
  • Other duties as assigned.

GENERAL EXPECTATIONS

  • Provide high quality customer service to internal and external customers.
  • Must work well under pressure, meeting multiple and sometimes conflicting deadlines.
  • Demonstrate cooperative behavior with colleagues and supervisors.
  • Participate in Quality Improvement and Performance Improvement Processes of the agency.
  • Complete all TGC required trainings on time.
  • Maintains a high level of professionalism while performing job functions.
  • Demonstrate flexibility and a willingness to learn.
  • This position is 100% onsite, in the state of Arizona.

EDUCATION/EXPERIENCE

Required:

  • Must be at least 21 years of age.
  • 1-year Behavioral Health Experience
  • Bachelor's degree in a field related to behavioral health - or one of the following:
  • Bachelor's degree and at least one year of full-time behavioral health experience
  • Associate's degree and at least two years of behavioral health experience
  • High School Diploma and four years of experience in the behavioral health field
  • High School Diploma and two years of experience in the behavioral health field and 18 credit hours in Sociology, Social Work, Psychology or a closely related field completed in the previous four years.
  • Ability to obtain and maintain Arizona Fingerprint Clearance Card.
  • Current driver's license and able to pass MVR screening.

Preferred:

  • Inpatient Behavioral Health Experience
  • Previous Case Manager Experience

Medium Work: Occasionally exerting 20-50 pounds of force or frequently exerting 10-25 pounds of force to lift, carry, push, pull, or otherwise move objects, including the human body; walking or standing to a significant degree.
