Care Manager

POSITION SUMMARY

The primary function of this position is to coordinate and provide mental health and/or substance abuse services for adult clients.

ESSENTIAL FUNCTIONS

1.  Monitors the progress of the client toward their goals.

2.  Assists the client in creating a crisis/safety plan if necessary.

3.  Participates in the development and monitoring of a Behavioral Health Service Plan (BHSP).

4.  The Case Manager will assist clients in obtaining needed services.

5.  Assists client in finding necessary resources other than covered services to meet basic needs.

6.  Communicates and coordinates client care with family members, behavioral and medical health providers and community resources.

7.  Assesses the progress and level of functioning of the client.

8.  Provides Life Skills Training if needed as evidenced by documentation in the client record.

9.  Facilitates Adult Recovery Teams to identify client needs.

10.  Advocates for the development of needed services.

11.  Manages caseload and completes required documentation in compliance with agency standards.

12.  Provides customer service to internal and external stakeholders.

13.  Participates in the Quality Improvement processes.

14.  Meet productivity agency standards.

15.  Complete all TGC training according to company policy.

16.  Willingness to learn.

17.  Performs other duties as assigned. 

KNOWLEDGE/SKILLS/ABILITIES

 Required: 

  • Bachelor's degree plus one year of related work experience OR- associate degree plus three years of related work experience OR- High School Diploma plus five years of related work experience OR- any other combination of education and work experience equivalent to 5 years as approved by Human Resources.
  • Current driver's license and able to pass MVR screening.
  • Must be 21 years of age.
  • Ability to travel to other clinic sites if needed.
  • Strong interpersonal skills.
  • Ability to obtain Arizona Fingerprint Clearance Card.

Preferred: 

  • One (1) year of case management experience.
  • Prior experience working with substance abuse and/or general mental health populations.
  • Experience working with adults with complex emotional needs.
  • Knowledge of available community resources.
