Community Health Worker

POSITION SUMMARY

The Community Health Worker (CHW) assists individuals in accessing needed treatment, services, and community resources. This role incorporates a trauma-informed, member-centered approach. The CHW establishes trusting relationships with members and their families while providing education, care coordination, advocacy, and assistance in navigating healthcare and social services systems. This is a grant-funded position working with pregnant and parenting women with substance use disorders.

POSITION DUTIES

  • Plan, conduct, and facilitate health education sessions using established curricula and educational materials.
  • Provide or assist in accessing services specified in the member service plans, including transportation.
  • Work individually with members to achieve goals outlined in their service plans.
  • Complete concurrent/collaborative documentation, including progress notes, screening notes, staffing notes, and group notes.
  • Participate in care team meetings and other clinical staffing to coordinate care with case managers and clinical staff.
  • Accompany clinical staff on home or community visits for outreach and wellness checks.
  • Assist in coordinating care between clinical departments and community agencies.
  • Provide ongoing follow-up, basic motivational interviewing, and goal setting with members and families.
  • Help members connect with transportation resources and provide appointment reminders.
  • Assist members in setting personal health-related goals and attending medical and behavioral health appointments.
  • Provide referrals for community-based services and ensure members have comprehensive and coordinated care plans.
  • Act as a patient advocate and liaison between the member/family and community service agencies.
  • Maintain confidentiality and compliance with HIPAA regulations.
  • Participate in job interviews for agency positions as requested.

  • This position is 100% onsite/TGC campus, in the state of Arizona.

EDUCATION/EXPERIENCE
Required:

  • High school diploma or high school equivalency diploma.
  • Current driver's license and ability to pass MVR screening.
  • Must be able to obtain and maintain an Arizona Level One Fingerprint Clearance Card.
  • Strong knowledge of community resources.
  • Ability to cope with emotional demands and secure appropriate support as needed.
  • Effective communication, active listening, and motivational interviewing skills.
  • Proficiency in using electronic medical records (EMRs) and other documentation systems.
  • Basic understanding of medical and behavioral health terminology.

Preferred:

  • Completion of a Community Health Worker formal training program (college or educational institution).
  • Experience working in a community-based setting for at least 1 to 2 years.
  • Experience working in a multi-cultural setting.
  • Medical terminology and/or healthcare background.
  • Bilingual (English/Spanish) preferred.
