POSITION SUMMARY
The primary function of this position is to provide therapeutic services to members. Maintains core values of integrity and ethical practice, professionalism, kindness, effective communication, and excellence in customer service.
ESSENTIAL FUNCTIONS
1. May provides services such as: intake, crisis or updated assessment, individual and/or group therapy, case management, psychoeducational services, family support, life skills training, health promotion, transportation, and others as defined by the plan of care (treatment plan).
2. Uses recovery-oriented, person-first language in all interactions, with a strong understanding and ability to demonstrate excellence in customer service.
3. Provides timely, professional and kind customer service to internal and external stakeholders.
4. Collaborates for coordination of care with internal and external providers and community partners such as law enforcement, schools, etc.
5. Participates in interdisciplinary case consultation, provides timely and effective feedback to co-workers regarding care and clinical documentation.
6. Facilitates treatment team meetings, makes appropriate referrals and assists members in finding resources.
7. Meets and sustains productivity standards and completes documentation concurrently or on the same business day and reviews progress and/or barriers in supervision.
8. Understands the business operations needs of the assigned department and agency related to hours of work.
9. Participates in the Quality Improvement processes of the agency when requested and available.
10. Completes all TGC required training timely.
11. Performs other duties as assigned.
DEPARTMENT SPECIFIC FUNCTIONS
Based on the assigned department, these additional duties or prioritized focus in the following areas will apply to the position:
1. Writes, updates, reviews and/or approves assessments and interdisciplinary care (treatment) plans.
2. Provides both individual and group therapy according to member and departmental needs.
3. Monitors progress of a member or family toward care objectives and discusses outcomes with supervisor(s).
4. Manages a caseload size and/or scheduling flexibility consistent with the acuity of those served and coordinates care for all assigned members, both with internal and external interdisciplinary team members.
5. Provides services at clinic and in community in locations such as member homes, schools, hospitals, detention centers, etc.
