POSITION SUMMARY

The Intake Clinician is part of a team of professionals who screen, triage, and complete clinical comprehensive assessments for new TGC members. The Clinician works closely with other team members and clinical programs, and must demonstrate collaborative and open communication skills. Must be flexible, adaptable, and have strong decision-making skills to prioritize service provision within a fast-paced environment. The position is cross-trained in the completion of crisis interventions and works closely with the crisis team. Intake clinicians complete assessments on The Guidance Center campus as well as in the community, including completion of assessments at Juvenile Detention Center, Flagstaff Medical Center, member homes and Coconino County Jail.

Essential Functions

1.  Complete comprehensive assessments, crisis intervention assessments, screenings, and triaging for any member(s) presenting to Crisis or Intake for services. Populations include: birth-five, children, adolescents, adults, and geriatric members presenting with mental health, developmental disabilities, co-occurring, and/or substance abuse issues.

2.  Formulates a thorough diagnosis.

3.  Demonstrates sound clinical documentation on assessments, crisis prevention plans/safety plans, behavioral health services plans, and contact notes that meet the needs of the member by identifying medical necessity. Clinician involves the member, family, and other natural supports during the assessments to identify the interventions that best meet the member's behavioral and mental health needs. All assessments are completed in accordance with AHCCCS and HCIC requirements.

4.  Clinical documentation must include an understanding and knowledge of psychopathology, crisis theory, and the primary modalities of psychotherapeutic interventions.

5.  Must be efficient and demonstrate an ability to multitask, engaging members in the assessment and treatment planning process while concurrently completing documentation and paperwork.

6.  Coordinates and collaborates with other various community agencies regarding crisis response services. When necessary, completes crisis assessments and interventions at the Coconino County Jail, Flagstaff Medical Center, Juvenile Detention, and/or in member's home/community. Willingness to collaborate with treatment team to provide support during crisis assessments. Makes appropriate referrals when needed.

7.  Provides crisis services to those members who present in need. Screens, triages, and implements immediate and follow up crisis intervention services and stabilization.

8.  Complete time closures of crisis episodes of care.

9.  Engages in crisis follow-up service plans including assisting in scheduling intakes, coordination with clinical teams, and providing appropriate community referrals

10.  Assist in obtaining and documenting member's financial and insurance information in the electronic health record.

11.  Function as an inter-disciplinary team in coordinating and advocating for the needs of the member.

12.  Ability to triage the daily needs of the department to manage scheduled intakes, walk-in intakes, and walk-in crises.

13.  Provide consultation and education to members (and families), as deemed necessary, regarding treatment.

14.  At all times, remain professional and deliver excellent customer services to both internal and external stakeholders.

15.  Perform other duties as assigned

16.  Majority of functions are within the completion of intake documentation, however, in certain circumstances the clinician will provide support to the crisis team. This includes when members presenting for intake are experiencing a mental health crisis, when appointments do not show and crisis needs additional support, or when crisis assessments are needed at Coconino County Jail.

KNOWLEDGE/SKILLS/ABILITIES

Required:

Masters Degree in a behavioral health related field.

Ability to obtain Arizona State Fingerprint Clearance.

Valid driver's license and ability to pass TGC Driver background screening.

Must be at least 21 years of age.

Must be licensed eligible at the associate level, and able to obtain licensure within the first 6 months of employment

Preferred:

Independently Licensed in the State of Arizona in one of the following disciplines: Social Work (LMSW or LCSW), Marriage and Family Therapy (LAMFT or LMFT), Counseling (LAC or LPC), or Psychology (Licensed AZ Psychologist). Approved by the Arizona Board of Behavioral Health Examiners as a clinical supervisor.

Experience in Intake, Assessment, Crisis, Title XIX Mental Health, and/or Substance Abuse Services.
